Practical guide

Create and publish your first form

Prepare an enrolment form, add the blocks the club needs, review it and share its link or QR code.

Quick answer

Create a draft form, add only the data and consent blocks you need, check the preview and publish it. You can then share its public link or QR code with families.

Public forms let you collect athlete and guardian details without transcribing them by hand. Each submission reaches the dashboard for the club to review.

Before you start

Decide what the form is for — a season, camp, renewal or another activity — and what information is essential. If you will link payments or a season, configure them first.

Also prepare the introduction, terms and consents the family must accept.

Ask only for data you need. Medical information, documents and other sensitive data require a clear purpose and appropriate permissions within the club.

Step by step

1. Create the draft

Open Forms and select New form. Enter the title, public address and a short description.

You can link a season. If you leave it blank, any configured charge starts based on the approval date, not the start of a season.

2. Design the header

In the builder, check the name and description. Add a poster if it helps people recognise the activity and check how the header appears.

3. Add the required blocks

Add blocks from the library. For example, you can request:

  • Athlete details.
  • Guardian or contact details.
  • Consents.
  • Documents.
  • Payment method or enrolment payment.
  • Custom fields and explanatory information.

Configure each block and arrange the steps so a family can complete the form without help.

Under Athlete details → Configure, you can enable Health card number. It is off by default and accepts letters, numbers and separators, up to 100 characters. You can make it optional or required. Request it only when necessary and add the corresponding consent under Consents.

Under Guardian / contact details → Configure, enable Secondary contact to request a name, relationship to the athlete and phone number. The relationship is free text; for example, “Aunt” or “Trusted neighbour”. One control enables all three fields and lets you make them optional or required. If they are optional, the family can leave all three blank; if they start filling them in, all three must be completed.

Both options are available in new and existing forms, for minors and adult athletes, with no country restriction. Save changes to apply them to future submissions. Previous enrolments keep the form and data they were submitted with. The secondary contact is contact information: they do not get app access and do not become a guardian or payer.

4. Review the settings

Open the form settings and check the type, dates, season and enrolment rules. If you include a payment block, set up the club’s payments first.

5. Preview as a family

Select Preview and go through the whole form. Check the mobile version, required fields and consent wording.

The preview includes changes that have not yet been saved, so you can spot issues before publishing.

6. Publish the form

Select Publish, then confirm with Publish form. The public link is fixed. The status changes from draft to published and the form starts accepting submissions.

You can copy the public link from the list. Open the row’s action menu and choose Download QR to view, print or download the code. Use it on the club website, social media, posters or in messages to families.

If something goes wrong

  • You cannot publish: check the builder’s message and complete the required block settings.
  • The public link is not what you expected: edit the settings while the form is a draft. Once published, the link is fixed.
  • The payment block does not work: first complete Set up online payments with Stripe.
  • You need to stop enrolments temporarily: pause the form; you can resume it without losing submissions received.
  • The campaign has ended: archive the form. It no longer appears by default, but its enrolments are kept.
