Complete an enrolment as a family

Fill in the club's public form, provide documents and consents, set up payment and keep the tracking reference.

Quick answer

Open the form shared by the club, complete each block with accurate details, attach the requested documents, review consents and payment, then submit. Keep the reference to check the status.

Public forms let you submit an enrolment without first signing in to the club dashboard. Each form can request different information, documents, consents and payment methods depending on the activity.

Before you start

Use the club’s official link and prepare details for the athlete and responsible people. Have the listed documents and a valid payment method ready if the form requires one.

Step by step

1. Check the club and activity

On the first screen, check the club’s identity, activity name, dates, places and terms. Do not continue from a link if you do not recognise its club or address.

2. Complete the requested details

Go through the athlete, guardian, contact and additional-question blocks. Enter current information and avoid abbreviations that could make the club’s review harder.

If the club asks for a Health card number, enter it with its letters and leading zeroes; it accepts up to 100 characters. Under Secondary contact, enter the person’s name, their relationship to the athlete as free text, and their phone number with its country prefix. The prefix selector in all phone fields includes every country in the world: find yours by name, code or prefix, or paste the full number with +. This option may appear for minors and adults. If the group is optional, you can leave all three fields blank; if you complete one, you must complete all three. This person does not get app access or become another guardian.

Under contact details, enter the email address and complete Repeat email. Both addresses must match before you can continue or submit the enrolment. Capital letters and spaces at the beginning or end do not affect the comparison. If you correct the first field, check the second one too.

This check catches typing differences; it does not verify that the mailbox exists or can receive email. A valid-format message only confirms the address structure. It applies to the contact for an adult athlete and to their guardian, and does not require club setup. The second field is not saved with the enrolment.

3. Attach documents

Under Document request, select Upload beside each document you want to provide. Check the formats and maximum size shown in that row; common formats are PDF, JPG and PNG. Attach one file for each requested document.

Sometimes, when you select Upload, a Security check window with a Cloudflare checkbox appears at the bottom of the screen. Tick it and the upload continues automatically. It does not appear for everyone; this depends on the browser and connection.

Wait for the upload to finish and for confirmation with the filename before continuing. If you chose the wrong file, use Replace or Remove in that row. Required documents must be uploaded before you can submit the enrolment.

4. Read consents and payment details

Read each statement before responding. If the form includes a fee or payment, check the amount, recurrence and method. Complete the provider’s secure flow when it opens. The enrolment payment goes directly to the club’s account for that enrolment. FlamaTech never charges families.

If you choose Card as the payment method for fees, you save it at this step without any charge. Your bank may ask you to confirm the operation: that screen shows the club’s name as the merchant and an amount of €0.00. If the bank sends you to another tab or its own app to confirm, return to the form tab afterwards to finish. What you have entered remains in that browser for 60 minutes, so you do not need to repeat the enrolment if the page reloads.

5. Submit and keep the reference

Review the final summary and confirm once. Save the reference and status link shown after submission; you can use them to check whether the application is pending, approved or needs attention.

If something goes wrong

  • The form is closed: contact the club; do not use an unverified alternative link.
  • The email addresses do not match: check both, especially the name and domain, then continue again.
  • A file will not upload: check that its format and size match the limits shown in that row. Changing the filename extension does not convert the file. Check your connection and select Upload again. If an allowed PDF, JPG or PNG within the limit still fails, give the club the form link, exact message and file size so it can investigate.
  • The security check could not be completed: select Upload again and, if the Security check window appears, tick the box. Do not reload the page; you would lose what you have entered. If it fails again, try another browser or allow challenges.cloudflare.com in your content blocker.
  • We could not validate your session: complete the security check if it appears and select the submit button again. You do not need to change cards; entered details and the authorised card are kept for the retry. If it continues, check that the browser can load challenges.cloudflare.com and contact the club.
  • You returned from the bank and the form seems to have restarted: open the same link in the same browser. You will see We restored what you had entered and continue at the step where you were. If the bank confirmed the card, it appears as saved; if not, enter it again without repeating the rest. Do not close the provider’s window during card verification: if the bank opens another tab or its app, confirm there and return here.
  • The bank shows a merchant you do not recognise: card verification and charges always appear under the club’s name. If you see another name, do not continue and alert the club.
  • Payment is pending: do not submit repeatedly; check the status with your reference. If We are checking the payment result appears, the previous transaction is being verified to prevent a duplicate. If the notice continues, contact the club and provide the reference; do not start another enrolment to pay again.
  • You notice an error afterwards: tell the club the reference and the detail it needs to check.
  • You cannot access the families’ app yet: this is expected. Access is created when the club confirms the enrolment, so no sign-in link is sent before then. Check the status with your reference; once approved, you will receive an email and can sign in.

Next step

When the club approves the application, sign in by following Accept the invitation and access the families’ app. For questions, use Contact the club from the app.
